LevelsRx is a technology platform that facilitates connections between users and independent, licensed medical providers, as well as state-licensed 503A compounding pharmacies.

• LevelsRx Does Not Practice Medicine: The company itself does not provide medical advice, diagnosis, or treatment. All clinical decisions, including the decision to prescribe medication, are made solely by the independent medical providers.
• No Guarantee of Prescription: Completing an online intake form or purchasing a membership does not guarantee that a provider will write a prescription. Prescriptions are issued only if the provider determines it is medically appropriate.
• Compounded Medications: You acknowledge that compounded GLP-1 medications prescribed through this platform are not FDA-approved, are formulated by state-licensed compounding pharmacies, and may differ from brand-name drugs.

2. User Responsibilities

By using our platform, you agree to provide accurate, current, and complete information regarding your medical history, current medications, and physical health during the intake process and all subsequent consultations. You understand that withholding information or providing false information can lead to severe health consequences. You agree to follow the treatment plan exactly as prescribed by the licensed medical provider.

3. Membership, Billing, and Cancellation

LevelsRx operates on a self-pay, monthly subscription model.

• Billing: By enrolling, you authorize LevelsRx to charge your payment method on a recurring monthly basis for your selected treatment plan (inclusive of medication, consultation, and shipping).
• Insurance: LevelsRx does not accept or process health insurance. All services are strictly self-pay.
• Cancellation: You may cancel your membership at any time through your account dashboard or by contacting customer support. Cancellations must be made before your next billing cycle to avoid being charged for the subsequent month. Once a prescription is processed and shipped by the pharmacy, it cannot be refunded.

4. Intellectual Property Rights

Unless otherwise stated, LevelsRx™ and/or its licensors own the intellectual property rights for all material on this website. All intellectual property rights are reserved. You may view and/or print pages from this website for your own personal use subject to restrictions set in these terms and conditions.
